If I set AA in Nvidia Inspector as indicated, but use MultiSamplesPerPixel=4 in fsx.cfg, what AA am I effectively running at?!

Here's the place to go, Dylan: Steve has dealt with this at great length, but to your question "Should I be dropping down to 4xS?" - you have to set up a scenario, such as Steve did at Bowerman - the fence, car roof, door, etc.. take a screenshot and study that pic. It may be not be "quite" so good, but the frame rate might increase. It depends what improvement you're looking for, or what degradation you find acceptable.
